Information you enter
The plan lookup is intended for public Medicare plan codes such as H5216-435-002. You should not enter names, Medicare Beneficiary Identifiers, Social Security numbers, dates of birth, addresses, medical information, or other personal or protected health information into the plan-code field.
The site does not require an account to use the plan lookup and does not ask you to submit member information in order to view plan records.
Technical and usage data
Like most websites, hosting, security, and analytics systems may process basic technical information associated with a request, such as IP address, browser and device information, requested pages, referring pages, timestamps, and similar diagnostic data. This information can be used to keep the site secure, understand aggregate usage, and diagnose errors.
The site's first-party event analytics are deliberately limited. The custom analytics dataset records an allowlisted event type, a normalized public Medicare plan code when an event relates to a valid plan, a broad traffic-source category, and a broad page type. It does not intentionally store the raw text from failed searches, full referrer URLs, names, member identifiers, account identifiers, or a custom cookie or session identifier.
Plan searches
A Medicare plan code is a public identifier for a plan, not a member identifier. Search activity may be measured in aggregate to understand which parts of the tool are being used and whether searches succeed or fail. Successful lookups may be associated with the normalized public plan code. Failed lookups are counted without retaining the text that was entered into the search field.
Do not place customer or member personal information in the search field. The lookup is designed only for Medicare plan codes.

Medicare Plan Lookup may use cookies or similar technologies for security, analytics, site functionality, consent management, and advertising. When Google AdSense is enabled, third-party vendors, including Google, may use cookies to serve ads based on a user's prior visits to this website or other websites.
