October 26, 2026: reviewed guide publication
Six plan-reading guides are now published after Joseph Lipari's October 24 editorial review. They cover plan identity, hospital schedules, premiums, separate medical and drug limits, employer-plan verification, and matching official documents. Their worked examples retain exact coverage years, source evidence, and review limits. This guide review does not represent human verification of every plan in the catalog.
October 26, 2026: member premiums and assistance
Five additional exact 2027 records now cite limited carrier evidence. H1486-001-000 and H2225-001-000 explain how Extra Help and MassHealth affect member premiums. H2293-004-000 links the carrier's subsidy premium table, and H2836-006-000 distinguishes member premium and hospital costs from CMS source figures. H0034-001-000 identifies prior-year medical examples in its 2027 summary. These dated explanations preserve the annual CMS records and do not determine eligibility or verify every benefit.
October 26, 2026: carrier cost rules and geographic alternatives
Four exact 2027 records now cite limited carrier-document comparisons and their scope. H4213-019-000 distinguishes Arkansas out-of-network cost alternatives and its combined medical limit; H1651-001-000 notes a foreign-emergency deductible exception. H1119-001-000 identifies the carrier's prior-year medical cost placeholders, and H2450-007-000 identifies the reviewed Minnesota document and drug-deductible scope. The annual CMS records remain unchanged. These checks do not verify every benefit, the full Evidence of Coverage, or individual eligibility.
October 25, 2026: carrier names and deductible context
A dated notice identifies the October 2026 CMS Landscape's HealthSpring marketing name for 40 exact 2027 S5715 records and directs readers to the current carrier document finder. The original annual record and benefit amounts are preserved. Pages display reported deductible-exempt drug tiers, and the quick lookup displays available drug deductibles. Dual eligible plan pages distinguish CMS figures from member liability; five exact annual records include cited carrier cost context. The original CMS zero-dollar medical cost-sharing designation is now displayed for all 739 flagged 2027 D-SNP records, with an explicit Part D exclusion. The lookup, comparison, and copied notes carry the relevant explanations. See the source and review limits.
October 23, 2026: advertising delivery
The ad integration was changed to use the supplied desktop/mobile banner sizes. This was an advertising delivery change; CMS plan values did not change. The current advertising status is described in the privacy policy.
October 23, 2026: official document pilot
A 12-plan UnitedHealthcare pilot added matching 2027 Summary of Benefits and Evidence of Coverage documents. Its source checks cover monthly plan premium, medical out-of-pocket limits, and in-network inpatient costs. This scope does not cover every benefit or group-specific entitlement.
October 22, 2026: source displays and accountability
The inpatient explanation was corrected for schedules without a reported charged-day count, while preserving the source amounts and coverage year. Employer-only county navigation was aligned with the public directory, Puerto Rico municipality labels and 2027 geography-source wording were corrected, and the named operator and correction process were added. Carrier document resources remain labeled with their verification limits.
How a future correction will be recorded
A record will identify the affected year, plan code or template, what changed, the supporting source, and the publication date. Reports containing personal information will not be published. Pending investigations are not described as completed fixes.
