A faster way to identify Medicare plans
Medicare Plan Lookup is a quick-reference tool for looking up 2026 Medicare Advantage and standalone Medicare Prescription Drug Plan records by their CMS contract, plan, and segment codes.
The site is designed to make common plan details easier to find during a time-sensitive lookup. For medical plans, that can include inpatient hospital cost sharing, ambulance, emergency room, maximum out-of-pocket, home health, and premium information when those fields are available in the CMS source data used by the site.
What a plan code means
A code such as H5216-435-002 contains a CMS contract identifier, plan identifier, and service-area segment. Some plans have more than one segment, so the segment can matter when identifying the correct record.
Codes beginning with S commonly identify standalone Medicare Part D prescription drug plans. A PDP result does not describe a person's separate medical or supplemental coverage.
Medicare Plan Lookup does not enroll members, determine eligibility, recommend coverage, or replace a carrier's current plan documents.
Independent site
Medicare Plan Lookup is independently operated. It is not affiliated with, operated by, sponsored by, or endorsed by the Centers for Medicare & Medicaid Services (CMS), Medicare, or any insurance carrier.
How to use the information
Treat results as a fast reference. Before relying on a benefit or cost-sharing amount for an enrollment, coverage, or financial decision, confirm it in the plan's current Summary of Benefits, Evidence of Coverage, or other official plan material.
