Confirm admission status and the applicable benefit

Ask the hospital or doctor whether there is a doctor’s order and formal inpatient admission. An overnight observation stay can still be outpatient care. Medicare Advantage costs can differ from Original Medicare; confirm the plan’s applicable benefit and network.

The task here is to read one reported hospital schedule. It does not determine whether a particular admission or every service is covered.

Work through one reported daily schedule

The retained 2027 CMS record for H5216-435-002 lists hospital tier 1 as $465 per day for days 1–5, then $0 per day for days 6–90. The source labels these intervals as a Medicare-covered stay.

These amounts describe the listed hospital cost-sharing schedule for an applicable covered benefit. A $0 daily interval does not make all hospital-related services free or confirm that every day is covered.

Days assumed to count under this scheduleListed daily amounts added
33 × $465 = $1,395
55 × $465 = $2,325
7(5 × $465) + (2 × $0) = $2,325

Start at day 1, apply the first interval only to the days within it, then apply the next interval to any remaining assumed days. This is arithmetic on the reported schedule, not a bill estimate. Separate professional services, observation, other benefits and the plan’s coverage conditions are outside this calculation.

Read the conditions alongside the dollar amounts

For this same retained record, the decoded CMS hospital fields describe the period as per admission or per stay, report no cost sharing on the discharge day, and mark authorization as required. Those fields explain why seven nights or a second admission cannot automatically be treated as the same seven chargeable days used in the illustration.

Match these reported conditions to the applicable Evidence of Coverage. Confirm how the plan defines the period and counts days, whether the applicable authorization was obtained, and which facility/network tier applies. The table does not determine additional-day or lifetime-reserve coverage.

Compare a daily schedule with a per-stay amount

A different 2027 record, H0609-059-000, reports a $395 in-network hospital copay per stay. Multiplying that field by seven days would change its reported basis. This is a separate plan example, not an alternative price within H5216-435-002.

Coinsurance has a percentage basis instead. Read the percentage and what it applies to; do not treat it as a fixed dollar copay. If alternative hospital tiers are shown, identify the applicable tier rather than adding the alternatives.
